Observed no handsink or hand hygiene program in place for facility in linen area. EHS provided PIC a copy of a hand hygiene program example via email during inspection.

Observed baseboards throughout rooms on floors 6, 4, and 3 with dust build up.Observed shower wands with pink build up in rooms 617, 610, and 309. Observed wooden furniture under removable glass tops in need of cleaning in rooms 500, 417, and 102. Observed broken closet door in room 512. Observed door of the 4th floor storage room with a broken hinge. Observed stained shower curtain in room 229. Observed stained window shades in rooms 311, 230, and 229. Observed rust where the shower door meets the shower base in room 512. REPEAT VIOLATION

15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. Observed no labeled carts used for soiled laundry.

15A NCAC 18A .1828 (d) Housekeeping carts shall be kept clean and stored to protect items from contamination. Observed housekeeping carts on floors 1 and 2 with chemical bottles hanging on the edge of the clean linen bins with spray bottle touching and/or directly over clean linen. Observed housekeeping cart on floor one with personal drink hanging within clean linen bin.

15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. Observed food residues inside a microwave in a hall near ice machine on first floor. Observed coffee maker has stain and residue on it in room 105. Need cleaning and keep clean.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed facility without test kit for their sanitizing solution in the laundry room. Obtain a kit for testing purposes.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed stain roller mattresses in storage room 4th floor when guest request. PIC took off the roller for reject item at the time of inspection.

15A NCAC 18A .1828 (e ) Soiled laundry shall be handled and stored separately from clean laundry using separate, cleanable carts or bags. Observed soiled linen stored in a green cart next to clean linens on shelves touching each other. Ensure to keep them separate and not touching to contaminate each other.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to avoid contamination of food and food contact surfaces, linen, and single-use articles. Observed packages of water bottles, boxes of toilet/facial tissues stored on the floor in storage room 5th, 4th, 2nd floor. Keep all item above the floor.

15A NCAC 18A .1824(i) Single-use articles may be used if discarded after each use. Single use articles may be stored and handled to prevent contamination. - REPEAT- Observed coffee filters and coffee cups stored on the floor in storage rooms on floors 6th, 4th, and 3rd floors.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. - REPEAT- Observed walls and pipe installation damage in housekeeping closets on 6th, 5th, 4th, 3rd floors. NOTE: no point deduction since last inspection dealt with ceiling damage in fitness center and one bathroom. All other physical facilities are in great repair. However, point deduction will occur if violation is noted again on next inspection.
