15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Wall damage in storage closet on 4th floor storage closet. Repaint wall/repair damage. 0 pts.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed dust on AC vent at the wall in room 460, 367. Need cleaning and keep clean. Observed food residues in microwave room 236, and rusted microwave in room 333. Observed broken or misplaced shelf inside cabinet in kitchenette area in room 460. Need cleaning, repair or replace as see fit.

REPEAT 15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to aoid contamination of food and food contact surfaces, linen, and single-use articles. Observed boxes of linens, single used items stored on the floor in the storage room 4th floor. Housekeeping manager is currently working on them, all items will be off and above the floor to prevent any possible cross contamination.

15A NCAC 18A .1824(i) Single-use articles may be used if discarded after each use. Single use articles may be stored and handled to prevent contamination. Observed boxes of single-service cups stored on the floor of storage room. REPEAT

15A NCAC 18A .1825 (c ) A handwashing sink, located to allow use by employees handling clean and soiled linen, shall be provided. This requirement shall be effective one year after the effective date of this Rule. Adjust nozzle on hand sink to provided hot water at all times. The faucet may turn at times and not provide hot water due to maintenance.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed stains on couches and chairs in several guest rooms.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed damage to walls in storage rooms on the fourth floor. Door frame was also damage with adhesive glue. Observed damage to walls in rooms 315, 312, and 436

15A NCAC 18A .1826 (i) Maintain premises free of insects, rodents, and other pests. Observed one worm-like insect in corner of wall in room 654.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed test kit for quat sanitizer with expiration date of September 2018. Replace with updated test kit for accurate testing.

15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observed (white) shower heads with visible build up; guest rooms 224 and 123. 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed couch cushion in guest room 224 with blue stain. Observed pillows with staining in guest room 338 and 123. Observed bathroom tub with staining in room 441. Observed mattress pads with set in stains in guest rooms 664 and 441.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed stained comforter in rooms 561 and 123. Observed debris on pull-out couch linen in room 561.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed broom stored above toiletries and linens on housekeeping cart. Observed clean single use products stored in contact with soiled, trash side of housekeeping cart.

15A NCAC 18A .1827(d) Sanitizing solutions shall not be stored in or dispensed from containers previously containing other poisonous or toxic materials. Sanitizing and disinfectant solutions stored in chemical bottles without differing labels or no label.

15A NCAC 18A .1826 (i) Maintain premises free of insects, rodents, and other pests. Dead cockroach in main lobby area floor. CDI- Cleaned.

Observed pink buildup on ice shoot of ice machine on 4th floor of main building. Also observed pink buildup on mouth piece of water fountain by main laundry room. 4-601.11(A)* Equipment food contact surfaces and utensils shall be clean to sight and touch. -Pf
