15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observed no test strips for QAC sanitizer. PIC ordered strips during inspection.

Observed broken filters in rooms 311,223 and 232. Observed cabinets with scuffs and scraps throughout facility. Observed black build up on bathroom walls over doorway in room 303

15A NCAC 18A .1824 (b) Equipment and utensils shall be kept clean and in good repair. Observed orange/pink build up outside of ice machine chutes throughout facility. Observed ice machine on third floor with an active leak, facility is aware of issue and are actively working to have leak repaired.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed brown staining on drapes in room 409.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed several stained ceiling tiles behind front desk/office area. EHS advises facility to ensure no active leak is occurring and to have stained ceiling tiles replaced/repaired.

Observed dumpster lids opened during time of inspection. 5-501.115 A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. Observed minor litter/debris behind dumpster area.

15A NCAC 18A .1824 (b) Equipment and utensils shall be kept clean and in good repair. Observed pink/orange build up inside the chute of ice machines located on floors 2-4. EHS encourages facility to increase cleaning frequency of ice machines throughout building.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed tile located on wall of shower cracked behind shower chair in room 124. Points de-escalated due to improvement from previous inspection report.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed stained ceiling over ice machine located on third floor. EHS advises facility to ensure no active leak is occurring and to have stained ceiling repaired.

REPEAT 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed all PTAC units in guest rooms (423, 402, 323, 308, 219, 202,102) having large dust and debris build-up on inside blower fan in need of detailed cleaning. (points not increased due to improvement for noted violations on prior inspection).

15A NCAC 18A .1823 (c ) Ice used for room service shall be manufactured from a water supply that complies with 15A NCAC .1700 rules and 15A NCAC 18C rules and shall be stored and handled in a manner so as to prevent contamination. Observed ice machines red and yellow buildup on the 2nd and 3rd floors. Note: Floors 3 and 4 are closed down. There are 82 rooms total and approximately 20 rooms on each floor. Checked 6 rooms.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed cracks in the ceiling along the showers in rooms 210, 207 and 205. Observed wall paper tearing behind the toilet in room 207.

Observed bathroom door damaged in room 418, pink shower heads in room 418 and 317, damaged desk and TV stand in room 303, cracked tub in 102, damaged wall behind chair in 102, water damaged ceiling in 201, water damaged cabinet under sink in 201, and a pen mark on the curtain in 219.
