4-601.11(A)* Equipment food contact surfaces and utensils shall be clean to sight and touch. -Pf Observation: EHS observed debris buildup in the ice machines on floor 4 and 2 in need of cleaning. REPEAT. Points not deescalated due to overall improvement.

15ANCAC 18A. 1824(k) A test kit or other device that measures the concentration in mg/L of sanitizing solutions shall be provided by the lodging establishment. Observation: EHS observed that the facility could not provide test strips for the quat sanitizer used.

15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Observation: EHS observed the shower head in room 825 in need of cleaning. 15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observation: EHS observed mattress in rooms 813, 502, 403, 339, and 211 that were stained. EHS observed pull out couches in rooms 813, 502, 339 that were stained. EHS observed curtains in rooms 825, 725, 502, and 403 that were stained. EHS observed a rip in the couch of room 602. EHS observed rips in the shower chairs in rooms 602 and 502. EHS observed a tear in the carpet near the bed in room 825. EHS observed lose shower handles in rooms 725 and 602.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observation: EHS observed stained linens in rooms 725 (mattress cover), 339 ( pillow ) and 211 (pillow, mattress topper)

15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. Observed laundry carts in laundry room not labeled to identify carts used for clean laundry and carts used for dirty laundry. CDI- PIC labeled carts.

Observed single service lids not properly stored on the housekeeping carts. **15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items.

Observed black residue in ice chute in floor ice machines.

Observed stains on gray chair in rooms 540 and 703. Observed shower cracked in room 838.

Observed celling peeling in hall near elevators on 8th floor, ceiling stained in 838, ceiling peeling around shower in 703, black residue in crevice/joint near shower in 603, black residue on ceiling of shower in 511, and holes in wall near shower and around socket (bedroom) in 401.

4-602.11* Equipment food contact surfaces and utensils shall be cleaned at necessary frequencies.-P Observed ice machines with black build up on floors 2 and 3. Recommend higher frequency of cleaning. Points not elevated due to improvements since last inspection.

Observed broom heads stored upright above clean linens on house keeping cart. Recommending storage position or order to prevent cross contamination of clean linens.
