15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed AC unit filter with dust buildup in room 504. Observed loose carpet in corner between bed and bathroom in room 201.

Observed doors open on dumpster. 5-501.114 Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place. Observed drain plug missing and cloth in place on dumpster.

5-205.15 Maintain a plumbing system in good repair. Observed water leaks in one of the linens room in the 4th floor. Needs to be repaired.
