Observed stain on ironing boards in room 1008. Observed lobby area furniture stained/ damaged throughout.

Observed base board chipping on the couch in room 805.

15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed pillow stained in room 910. Observed mattress cover torn in room 1510.

15A NCAC 18A .1828 (d) Items on housekeeping carts shall be arranged in a manner to prevent cross-contamination between soiled and cleaned items. Observed broom stored near cups on housekeeping cart. Observed chemicals stored with cups on housekeeping cart. Chemicals and broom relocated away from cups.

15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. Observed ice machine chutes on 13, 11 and 3 with pink and black build up.

15A NCAC 18A .1824(i) Single-use articles may be used if discarded after each use. Single use articles may be stored and handled to prevent contamination. Observed cases of toilet paper and/or tissues stored on floor in several storage rooms throughout facility.

Observed sink in the laundry room with no soap or paper towels. Staff uses restroom located near the laundry room to wash hands. Informed staff that the sink in the laundry room should be used for handwashing. **15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.

15A NCAC 18A .1826 (c) Sinks, vanities, toilets, and showers in guest rooms shall be cleaned and sanitized between guests. Per conversations with several housekeepers, contact time or incorrect chemical being used for sanitizing vanities. CDI: EHS explained to PIC proper required procedures. Observed desks needing cleaned in 1415, and 1210. Observed damaged desks throughout but particularly in 1313, 1114, 1012, 912, 811, 713, 504, and 412. Observed headboard stain in 1313. Observed stained curtains in 713 and 710. Observed chipped furniture in 1313, 1212, and 310. Observed bedside drawer in need of cleaning in 1212 and 504. Observed stained couch in 1212. observed track for sliding glass at desk in 1210 with debris. Observed broken reading light in 1114. Observed stained pillow in 1210. Observed cracked ice lid in 1212. Observed visibly soiled ice lid in 1212. Observed debris on trundle mattress in 1415.

Observed door thresholds on 12th floor in need of painting.Observed doors to storage rooms in need of painting. Observed slippery floor in 1313. Observed guest washing machine gasket with build up.

15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. Observed carts in laundry for linens unlabeled or with very faded labels.

15A NCAC 18A .1827(d) Sanitizing solutions shall not be stored in or dispensed from containers previously containing other poisonous or toxic materials. Observed sanitizer stored in bottles that previously had other chemicals per labeling. 15A NCAC 18A .1827( c) Household cleaning agents such as bleaches, detergents, and polishes shall be used and stored according to manufacturer's recommendations. Observed several containers of chemicals mislabeled or not labeled at all on house keeping carts. Observed unlabeled fabuloso chemical on several carts. PIC stated this is not a product the facility uses.
