REPEAT 15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair.- Observed soiled ice machine in the chute in need of cleaning on 2nd and 1st floor. Keep clean.

General Comment: 15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.- Observed no paper towels and soap available in the dispensers at employees break room sink. Ensure to provide all hand washing supplies at all hand sinks at all time.

REPEAT 15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair.- Observed ice machine needs detail cleaning in the chute on 4th, 3rd, and 1st floor. Observed soiled microwave need cleaning in 225, 223. Clean and keep clean.

5-501.15 Maintain refuse, recyclables, and returnables receptacles with tight-fitting lids, doors, or covers.- Observed dumpster door left opened when not in use. Keep doors close to prevent insects/rodents harborage. 5-501.114 Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place.- Observed one dumpster without drain plug. Obtain a plug and keep in place.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to avoid contamination of food and food contact surfaces, linen, and single-use articles.- Observed supplies (tissues, cups) store on the floor in storage room 2nd and 3rd floor. Store all supplies off the floor at least 6 inches to prevent any possible contamination.

15A NCAC 18A .1824(b) Equipment and utensils shall be kept clean and in good repair. Observed ice shoot on 4th floor with pink buildup, in need of cleaning. EHS observed that all other ice machine shoots on each floor were clean, only one on 4th floor needing cleaning.

15A NCAC 18A .1826 (h) Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Observed childrens toys underneath ADA room bed, in room 223. Observed loose toilet seat in room 203.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed stained carpet in room 425. Observed stained carpet in room 305 and just outside of room 305 on carpet in hallway. PIC stated that facilities has been notified, both of these areas have already had work orders placed. EHS observed overall that physical facilities are kept clean in good repair, these were only rooms with stained carpet during inspection. Observed loose toilet seat in room 203, PIC stated work order was already placed for this.

15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair. Observed loose debris under headboards in rooms 443 and 223. Observed storage of items on floor in manager storage room on 2nd floor. Floor inaccessible to be cleaned.
