Observed ceiling cracking above ice machine on 4th floor. Observed ceiling peeling outside of closet in 402. Observed womens bathroom door cracking at corner. REPEAT. Points not escalated due to improvement in items from last inspection.

Staff stated they place plastic bags in carts when switching between clean and soiled laundry. Soiled laundry carts need to be labeled for soiled laundry use only.
