15A NCAC 18A .1826(a) Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.- Observed no soap and paper towels available in the dispenser at hand sink in laundry room. Ensure to provide all hand washing supplies at all hand sinks at all time. REPEAT

Observed chipping furniture in room 101, bathroom light not working in room 205, loose toilet seat in 205, and stained shower curtain in room 313. Repair needed. Observed soiled/stained/damaged on tubs/showers/hand sinks in room 301, 207, 236, 132, 120. Cleaning/repairing needed. Observed caulking needed around hand sink in room 207. Observed dusts/debris in drawers, cabinets in room 313, 301, 309, 207. Observed missing closet door knob in room 301, 103. Observed minor mildew on AC unit in room 107. Observed dusty on AC filters on second and third floor hallway. Detail cleaning needed, repair and replace. Keep all fixtures in good repair and clean. - Points remain the same at this time.

General Comment: 15A NCAC 18A .1826 (f) Physical facilities shall be kept clean and in good repair.- Observed damaged floor in need of repair in the vending machine room 1st floor. Observed damaged/holes/mildew growth on ceiling above shower in room 236, holes on wall in restroom 132, stained ceiling in vending machine room 1st floor. Repair needed.

REPEAT 5-501.15 Maintain refuse, recyclables, and returnables receptacles with tight-fitting lids, doors, or covers. Observed dumpster door left open when not in use. Area around dumpster need cleaning and keep clean and neat. Point remain the same due to improvement.

15A NCAC 18A .1827(g) A storage area shall be provided for building and ground maintenance tools and supplies and stored in a manner to avoid contamination of food and food contact surfaces, linen, and single-use articles.- Observed cleaning supplies boxes (toilet tissue, linens, pillows, etc..) stored directly on the floor in closet 1st and 2nd floor. Store and keep all clean supplies above the floor at least 6 inches at anytime.

Detail cleaning needed and keep all furnitures and fixtures clean and in good repair.
