Observed build up to bottom of shower curtain liner in bathroom located to right of facility. Clean/replace.

15A NCAC 18A .1608 (a) The interior walls and ceilings, including doors, window, and window trim, shall be kept clean. Observed ceiling peeling around water sprinkler in bathroom 2. Repair.

15A NCAC 18A .1618 (a) All food service equipment and utensils shall be kept clean. Observed minor debris on flooring of reach in freezer. Clean.

Observed black microbial growth on the ceiling in the bathroom. **15A NCAC 18A .1608 (a) The interior walls (doors, window, and window trim) and ceilings and attachments (light fixtures, fans, and vent covers) shall be kept clean.

Observed knob missing on dresser in bedroom 6. Knob needs to be replaced. Observed brown staining on dresser in bedroom 2. Dresser was cleaned. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed clothing items stored on the floor in the closet in room 1. Items needs to be off of the floor. **Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Observed the linoleum torn in front of the dryer in the laundry room and in the small restroom near the toilet. Needs to be repaired. **The walls and ceilings of all rooms and areas shall be kept clean and in good repair. 15A NCAC 18A .1608.

Observed toilet seat leaking with towels around the base, toilet seat loose and black build-up on shower curtain in bathroom on the left side of the residence. Needs to be repaired. Observed dresser drawers off track in last bedroom on the left (left side of residence), handles missing on dresser and linen stained in last bedroom on the right (left side of the residence) and dresser handles loose in first bedroom on the left (left side of the residence). Observed dresser drawers off track in last bedroom on the left (right side of the residence) and mattress staining. Observed couch surface worn in Living Room. **All furniture, mattresses, curtains, draperies, and other furnishings shall be kept clean and in good repair. 15A NCAC 18A .1617 (a).

Observed shed outback with large whole in the roof and structure leaning. Needs to be removed if not used. **Rooms or spaces which are provided and used for the storage of clothing, personal effects, luggage, necessary equipment and supplies and for items not in routine use, shall be kept clean. 15A NCAC 18A .1616 (a).

Observed linoleum cracked near toilet in small restroom and in laundry room near the dryer. Needs to be repaired. **All floors shall be easily cleanable and shall be kept clean and in good repair. 15A NCAC 18A .1607.

Observed two bedrooms with no working light, last bedroom on the left (right side of the residence) and first bedroom on the left (right side of the residence). **All rooms shall be well lighted by natural or artificial means. 15A NCAC 18A .1609 (a).
