Note- MAIN DRAINS EXPIRE 12/2027 IN MENS SPA. REPLACE PRIOR TO EXPIRATION AND SUBMIT DOCUMENTATION TO THE HEALTH DEPARTMENT TO PREVENT SPA PERMIT BEING SUSPENDED.

Observed main drains checks not being recorded every day. These checks are there the majority of the time and are missing infrequently so no permit action taken. Ensure main drain checks are recorded in log book daily.

CPO Chase indicated the facility switched to a new corporate form and there is no spot on the new form to record main drain safety checks.
